Licensed Vocational Nurse (LVN) Hospice

American Care is looking to add a full-time LVN to our team. Here at American Care you can be a part of a compassionate team of healthcare workers. American Care is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. We also provide home based palliative care for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia.

Hospice LVN Essential Functions:

  • Provides direct patient care as defined in State Nurse Practice Act.
  • Responsible for delivering nursing care to patients as directed by the registered nurse and the physician’s plan of care.
  • Responsible for performing services which are planned, delegated, and supervised by the registered nurse.
  • Responsible for making decisions based on individual educational preparation and experience as a licensed practical/vocational nurse.
  • Responsible for frequent and timely communication to the appropriate registered nurse, supervisor and other staff members regarding the patient’s condition. Communication includes participating in case conferences and other more immediate methods of communication when indicated.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
  • Provides respectful and compassionate care to all patients/families/caregivers.
  • Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.
  • Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
  • Contributes to agency’s process to develop and implement improvement strategies.
  • Collects and reports data accurately, completely and in a timely manner.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal, professional growth and development.
  • Completes other assignments as requested and assigned.
  • May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.

LVN Minimum Education & Experience Requirements:

  • Graduate of an accredited vocational school for LPN/LVN and licensed to practice as LPN/LVN in the state in which employed.
  • Twelve (12) months experience in an acute care environment.
  • Two (2) year of hospice care or SNF experience preferred.
