Recognizing that each patient and family deals with illness and grief differently, the hospice care plan is tailored to meet those varied needs, while, at the same time, addressing the symptoms, need for pain management and all other technical aspects of an individual’s care. The comprehensive plan of care is directed by the patient and carried out by our hospice team, which may include, but is not limited to:
